I am using Forge 6.4.1.426 alone, no other mods. Re-downloaded twice and checked the Over-asked questions and found nothing. All crash reports are the same.

You have other mods.  They are not for Minecraft 1.4.5.

    • They were already updated, and just to double check I even did a cleanup and fresh update from that same page. I'm quite sure drivers are not the problem here. 
    • i tried downloading the drivers but it says no AMD graphics hardware has been detected
