Okay so I have all the major mods (Redpower2, Buildcraft 2.x, Industrilcraft2, Railcraft, Equivelant Exchange etc) To make my life easier as a server owner, i manually assigned all my block ID's nicely in the extra block ID space Before forge had the 4096 on my server we were still playing with the old ID's and not using any 4096 fix as we would have to start a new map. Then came forge with the 4096 fix and we thought, yes lets do it, and start a new map. So i load all my custom set ID's and a new map A few hours in my friend attempts to place a coke oven brick from railcraft and crashes finding this odd i start testing it out, and with the other mods Well what did i know, when we grew rubber trees from IC2 we would crash, when we placed any of the microblocks from redpower we would crash So I stripped out everything from my server an loaded up just redpower and still we crash I moved the ID's back into the 0-255 range, and didn't crash. I can conclude that there is a problem with tile entities and the 4096 fix in forge when using SMP At no time does the server crash it is perfectly fine, blocks are places when we log in Here's a crash log from what is happening, with the list of mods loaded I hope this is everything you need, i feel this is quite a big drawback/problem with the code.
