Game keeps crashing whenever someone joins

We just decided to start over since we had a few troubles from the start but thanks for the help -
Whenever anyone tries to join the server, the server crashes. The server was functioning before but then after teleporting using Xaeros_Minimap_1.17.4_Forge_1.12. This is my first modded server that I've hosted. Here is the crash log: https://pastebin.com/Bjm45Bgg
