  1. Disconnecting after joining modded server

    The most common culprit. A mod on your end or your friend's end is conflicting with something else, causing the game to freeze up and eventually disconnect you. A particularly demanding mod might be causing lag spikes that lead to disconnection. Ensure both you and your friend are using the exact same versions of Forge and all the mods. Even a minor version difference can cause problems.
