Game crashes after a while

So here's the problem:

 

When I play with for example Galacticraft, my game crashes after about 5 minutes. This issue is not limited to Galacticraft, it happens basically every time I try to add a new mod and also with the mods I'm currently playing with, however it takes a longer time for it to crash.

I will add another crashlog for my current mods after it crashes again.
