Vanilla Attributes

A while ago Attributes were added in vanilla Minecraft and attribute modifiers allowed people to do cool things like give you boots that increased your speed or a chestplate that ignored knockback or a hat that gave you extra hearts.  Sadly, this feature lounges mostly forgotten by all but the most elite mapmakers. How should I go about adding attribute modifiers to a custom item?  For this example let's just say I want an item that does nothing but have the attribute modifier for 200% speed while held.
