Minecraft black screen

I recently downloaded forge universal and managed to install it on fresh downloaded minecraft 1.4.5

i installed forge 6.4.0 universal and when i start MC i can see small window saying that forge is downloading etc.

Then i get black screen. No crash report. Just stays black. Here is the log.
