Solved - Very low TPS using Forge SP, Vanila & Optifine SP okay

I'm getting 5-7tps when using forge in a single player world.

/forge tps shows I'm getting DIM1 with a very high latency

Things I've tried:

Increasing/decreasing allocated RAM (currently at 4gb out of 12 system)

Reducing render distance from 12 chunks to 5.

Moving to other parts of the world/dimensions.

Disabled/removed all mods so only Forge is running.

Loaded Optifine through Forge.

Disabled all settings in Forge that I can.

 

Annoying thing is, due to the observer constant power bug I have to use Forge, where as running the launcher through Optifine without Forge It works smoothly.

There has been people with the same issue but it has always been caused by another mod generating too many entities.

My worlds spawn chunks is where the majority of the automated farms are, but they aren't creating excessive entities. (SP vs MP farms)

 

CPU and RAM are not even close to 50% usage. JAVA, Minecraft and Forge running latest versions.

 

Any ideas?

 

Edit - I'm running Xisuma mob head pack which is independently running in the back ground, I'm going to disable it and see if it makes a difference.

Edit 2 - Bloody Xisuma, he had a constant loop running updating the game every time a mob is in the area. Still strange it happens only under Forge not SP or Optifine.
