Weird visual glitches on Forge 1.8.9

Ok So every time I boot up minecraft there is these weird visual glitches that start on the home screen. The Mc version, the version of optifine, and the number of mods and stuff that's in the right hand corner are now on the left of the screen and in the background where the resourcepack background should be. Then ingame, It starts off fine then my hearts start duplicating themselves, and the most obvious one is that it makes a little square where the visuals are fine, but on the outside of that little square its all black. I cant get a screen shot of this because it only happens when Im full screened. Any suggestions on how to fix this?
