here is simple video

so i play my game for about 2 hrs. when i restart minecraft(i have 2 diamond pickaxe)  play a short time(i  put in chest), and close save world reenter it, my charactor will reset to i open minecraft(i have 2 diamond pickaxe)

don't know why this happen  

 

 

My forge is 1.12.2-forge1.12.2-14.23.5.2768

i also try it at forge-1.12.2-14.23.5.2814

still happen this issue when i remove all mods except forge
