[1.12.2] Item textures aren't working

I've recently run into an issue with Minecraft Modding via Forge. I was able to create a basic item, but when I try to add a texture to it I get this error.

I've been trying to figure this out for a while, and can't figure out why it's happening, even though the file structure looks fine to me.
