I have set up a 1.16.5 - 36.1.0 Forge Server on a VPS with 4 CPU Cores and 8GB RAM.

Without mods and 1 Player it max out one CPU Core to 100% and uses 3,5GB of RAM. But still has problems with tickspeed. With the MCMT Multithreading Mod installed, all cores are around 90%-100%, with same RAM and still it lags.

Is this normal?

Multithreading is not a magic solution to lag, there is no magic solution to lag.

To be able to give you any more tips I'd need to know the mods you're using and your full system specs, including disk IO, as well as the nature of the lag

The only official source for Forge is https://files.minecraftforge.net, and the only site I trust for getting mods is CurseForge.

If you use any site other than these, please take a look at the StopModReposts project and install their browser extension, I would also advise running a virus scan.
