Update your AMD/ATI drivers - get the drivers from their website - do not update via system -

Then this issue is caused outside of Minecraft Check for driver and Windows updates -

Start with allocating more RAM - 8 GB is not enough Also reduce the render distance from 12 to 10 or 8 -
I have the same problem on the same pack, I haven't tried multiplayer but singleplayer doesn't show recipes, I am on version 1.11.1
