Your graphics driver crashed. Remove Optifine and / or Shadersmod if installed. If that does not help disable the loading screen as described in the EAQ.

Could you please post a link for the part you mentioned in the EAQ

(this is the only possible solution i havent tried)

disable the loading screen by setting "enabled" to "false" in .minecraft/config/splash.properties.

 

Updating your graphic driver can also fix the problem.
