Weird crash

Whenever I load a world or log into a server, it looks like this:

If the video isnt uploaded by the time someone see's this, basically whats happening is when I load a world or log into a server, the blocks flicker, and often look like flashing triangles.

 

Oh and yes I know there are conflicting id's and biome ids etc...
