Not sure whats really going on, I thought the error would be the 0xC0000BB8 but not sure if thats exactly it.

The 0xC0000BB8 looks like an issue with your windows configuration.

It is unable to query some performance information. These are just warnings (it is not the error code 1), but you still might want to fix this?

 

Your crash is caused by you having the wrong version (or missing) mods

The one about forge is a mod wants a different version of forge to what you have installed. e.g. create needs at least forge 40.1.60
