After some time playing on the server, the server lags, or when someone teleports, many mobs become 'glitched' and become invincible. Many of our mob spawners are broken and full of glitched mobs, and a few players are invincible as well, and the only fix I could find was to reset their playerdata. Server restarts won't fix anything either. a few of the mobs (mostly pigs) completely negate falling, and all of these mobs won't despawn. I figured this may be due to a performance mod installed (ie. canary, smooth chunk, distant horizons, etc.) but I'm not sure. Any help is greatly appreciated, and thank you in advance.

There's no error in that log except some periodioc warnings about 5 second server thread ticks. They should take 0.05 seconds.

You can use a mod like: https://www.curseforge.com/minecraft/mc-mods/spark to look for what is causing lag. Read their wiki/docs to figure out how to use it.

Boilerplate:

If you don't post your logs/debug.log we can't help you. For curseforge you need to enable the forge debug.log in its minecraft settings. You should also post your crash report if you have one.

If there is no error in the log file and you don't have a crash report then post the launcher_log.txt from the minecraft folder. Again for curseforge this will be in your curseforge/minecraft/Install

Large files should be posted to a file sharing site like https://gist.github.com  You should also read the support forum sticky post.
