Perhaps related to using Task Manager to kill MC after a lag spike. This has happened before, but I don't remember how I resolved it.

sadly, with the massive amount of mods you have its damn near impossible to determine who is the culperate.

You're gunna have to split your mods and try debugging it, sorry dude cant help you more.
