It seems like your AMD graphics driver might be causing the trouble. I'd suggest updating that driver first since it can often fix these kinds of problems. If that doesn't do the trick, maybe try reinstalling Minecraft and Forge, just to make sure everything's fresh and clean. Also, double-check your Java version is up to date, as Minecraft can be picky about that. And, you know, it might not hurt to run a quick malware scan, just in case something sneaky is messing with your game.
