I see a stop command halfway through. Start it and don't do anything until it's stopped itself, then send debug.log as described in my signature below. -

Press Windows Key + R to get a run prompt, into that type cmd and press enter. That will bring up the command prompt. -

Open a command prompt and type java -version please. -
The jar doesn't actually go in the versions folder This looks correct. If you don't have a debug.log could you provide a launcher.log launcher_log.txt? It would be in .minecraft . Note: I'm going off of Windows experience here so I don't know if Linux even has a native MC launcher or is still using the old Jar launcher, the file might not even exist.

Your version of Xaero's minimap is very outdated and the file name doesn't match the one I get from Curse, that worries me slightly. Please check out the StopModReposts project for your own good.

Crayfish's furniture mod has a bug at the moment, it's been fixed in development but hasn't been fixed in release yet. Workaround is to disable the Furniture Mod welcome message from the server, in cfm.cfg -

Can't exactly speak for Optifine, dev tries to improve MC's rendering engine but it comes with its own bugs and doesn't always work. -
Try updating your mods.

Try removing shaders mod and possibly optifine. -
Mr Crayfish's Furniture mod has a bug in it that's been fixed in development but hasn't been fixed in a release version yet, the workaround is to disable the welcome message in the server's configs, cfm.cfg is the furniture mod config file.

If you are on http://files.minecraftforge.net which is the ONLY place you should get Forge from1, the Jar installer is the one that just says "Installer", rather than "Windows Installer" 1Note that any other place that claims to supply Forge may be trying to get you to download malware and blame it on Forge. I'm not kidding, people have come here complaining that the Forge installer installs unwanted apps when they downloaded a generic installer with mocked up Forge branding from some sketchy site.

Unless you are putting the instance somewhere to keep it separate from another instance the default will suit you fine. -

You set the Game Directory to the mods folder? Please note that the Game Directory is where those folders (config, logs, mods, resource packs, and saves) are kept, this is what allows you to keep separate instances/profiles of Minecraft for different versions and/or modpacks without constantly switching the files around. -

If you plan on running Forge servers for MC1.12.2 and prior you'll need to run them with Java 8 as well, I believe Gradle uses whatever JDK you point to with the JAVA_HOME environment variable. For example, mine is D:\Program Files\Java\jdk1.8.0_152
