Uninstall mapwriter, it's poorly coded. Forge mods should NEVER use net.minecraft.src.ModLoader, it's for compat with ML mods.

That depends. If you put all your mods in gamedir/mods, then no. If you use gamedir/mods/minecraftversion , then yes.

Redownload your forge installer, this has been fixed.
