If you installed with the installer that file will exist and you would not get a 404. This is exactly the error you are SUPPOSED to get if you did not install correctly. Whatever setup you have you are not installing Forge to the correct folder.

That is a normal play session lasting ~1.5 hours. The errors about Item Entity's is that something is spawning a Entity in the world for a item, without telling us what Item it is. There is a mod there somewhere doing it, or a extremely laggy server/client.
