If you're running the server there isn't a need to merge the files. Just run java -jar yourforgezip.zip .

Only forge should be in the minecraft.jar, no other mod (except those that explicitly say so). What is minecraft1.4.6.jar? Please post your logs. I use mcpatcher which auto installs and uninstalls mods logs Don't use mcpatcher. Use multimc, it's better.
